NOTE: Contract Cancellation Rights. You may cancel the remainder of time on your six month contract if you move your residence or your place of employment more than twenty-five miles from the Fitness Center or provide staff with a doctor's order that you cannot physically receive Wildcat Fitness Complex services. New membership is not effective until the PAR-Q health form, Wildcat Health & Fitness Agreement are approved and signed.
